Proxy Access for Online Services

We offer proxy access to online services for patients who require support in managing their healthcare, such as children, elderly relatives, or individuals with specific care needs.

Proxy access allows a trusted individual (e.g., a parent, carer, or legal guardian) to act on behalf of a patient to:

  • Book and manage appointments
  • Request repeat prescriptions
  • View medical records (where appropriate and with consent)

To apply for proxy access, please contact the practice. We will provide a Proxy Access Request Form, which must be completed and returned along with proof of identity for both the patient and the proxy.

Please note:

  • For children under 11, a parent or guardian may be granted full access by default.
  • For patients aged 11 to 15, access will be considered on a case-by-case basis, and the child’s consent may be required.
  • Patients aged 16 and over must provide explicit written consent unless there are exceptional circumstances.

All proxy access requests are assessed in line with national guidance and patient confidentiality standards.
